Yeah, the screen faded when that Fake-Riku walked towards him...
And seeing as though that's the last time you see him, then I think he did die.

Yeah he died a horrible death for his panel is broken in KH2.

The reason his panel was broken was so they did not have to reveal his weapon to you.

Well, he never really fought, and we dont know his weapo, yet nomura said he was designing his weapon at the same time he said he was making the faces of the knights...clue ?
